Our team final paper is titled Chronic Disease Management Information System. The paper looks at enhancing communication within the healthcare setting and thus improving chronic disease management. We analyze that chronic care patients who are not managing their condition are often seen within the emergency department or admitted to an inpatient setting. To both improve the quality of care for these patients by allowing them to more effectively manage their care through ambulatory visits and remote care.
The paper looks at the stakeholders, mission, requirements, information flow and architecture, potential challenges, and information standards like HL7. By understanding how data is transmitted, one can understand how to enhance communication and drive improved telecommunications.
The basics of database applications including system algorithms and "if this happens, then do this" logic is applied to support remote care management.
